Insurance

We accept most major insurance plans at our practice. Please call the office to speak with one of our knowledgeable staff members about your individual coverage.

Payments

All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which are usually taken care of in advance.

We accept cash, check, Visa, MasterCard, American Express, and Discover credit cards.

Cancellations

In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.
